SHBG sets how much of your testosterone is actually free to do anything, which means it can explain a total testosterone that looks fine and feels wrong, and one that looks low and is nothing of the kind. It is also made in your liver, so a low reading is among the earliest things blood chemistry shows when metabolism starts drifting. Two answers, one line item.

What is SHBG?

SHBG, or sex hormone binding globulin, is a liver-made protein that binds testosterone, DHT and estradiol in your bloodstream and controls how much of each stays free to enter tissue. Only 1 to 4 percent of circulating testosterone is free. Because SHBG production tracks liver fat, a low level also functions as an early metabolic signal, though it flags something to look into and diagnoses nothing.

What SHBG Actually Is (and Why It's on Your Panel)

SHBG is a transport protein your liver makes that binds androgens and estrogens with high affinity and limits how much of them can diffuse into target tissue. So what does SHBG do in practice? It decides who gets in. That is the SHBG meaning your report leaves out, and it is more consequential than it sounds.

It does not bind everything equally. Dihydrotestosterone (DHT) sticks hardest, at roughly a 1 nanomolar dissociation constant; testosterone binds with about five times lower affinity, and estradiol about twenty times lower. So a change in SHBG does not move all three hormones by the same amount.

What circulates, then, is three separate pools:

  • Free: 1 to 4 percent, unbound and active in tissue

  • Albumin-bound: 33 to 54 percent, held loosely enough to let go in a capillary

  • SHBG-bound: the remainder, held back from tissue

Now the part the title promises. One reason nobody explains this number is that no guideline puts it on every panel. The Endocrine Society calls for a free testosterone measurement, which needs SHBG to produce, in men whose total testosterone sits near the lower limit of normal or who have a condition that alters SHBG.

If you have never seen SHBG on a report, it is because that panel did not run it, which is its own piece of information.

Free vs. Total Testosterone: The Number That Changes the Answer

Total testosterone counts everything in the tube, only the free fraction can act on tissue, and SHBG is what sets the split between them. That is the free testosterone vs total testosterone distinction in one sentence, it is the free hormone hypothesis underneath, and it has been tested directly.

Researchers gave mice a human SHBG gene, which mice do not normally carry as adults. Total androgen and estrogen concentrations climbed. Free testosterone, measured by equilibrium dialysis with mass spectrometry, did not move at all, and hormone activity on reproductive organs went down. SHBG-bound testosterone is genuinely held back from tissue. One caveat worth keeping attached: that experiment ran in transgenic mice, not in men.

Here is the piece that makes everything downstream make sense. Your body defends free testosterone. Push SHBG up and free testosterone dips, luteinizing hormone rises, testosterone production increases, and free testosterone gets pushed back toward where it was.

High SHBG therefore tends to travel with a high total testosterone, and the compensation is not always complete.

Which is why the relationship between SHBG and free testosterone is not a simple ratio, why SHBG and testosterone have to be read together, and why total testosterone on its own gives you half the answer.

What Low SHBG Means: Including the Metabolic Warning You Shouldn't Ignore

A low SHBG is one of the earliest blood signals that insulin resistance is developing, and it is a reason to look closer rather than a diagnosis of anything. The evidence behind that sentence is unusually good for a single lab value.

Three findings, each with its population attached:

  • Metabolic syndrome: among adult men in NHANES III, each log-transformed standard-deviation increase in SHBG came with a 43 percent lower prevalence.

  • Diabetes risk: pooled analysis found men above 28.3 nmol/L of SHBG carried 52 percent lower risk of type 2 diabetes than men below it.

  • Timing: SHBG changes appear to precede clinically evident abnormalities in glucose handling, which is what earns the word "early."

Then there is the genetic evidence, which is where SHBG stops looking like a bystander. Using SHBG gene variants as a natural experiment, researchers put the odds of type 2 diabetes per standard-deviation increase in SHBG at 0.29 in men, 95 percent confidence interval 0.15 to 0.58. That points past correlation toward SHBG sitting in the causal chain.

Two honest qualifiers, because they are what make the finding hold up. Those variants explain only about 2.2 percent of the variation in people's SHBG levels, so the genetic signal is real and small.

And a 2024 analysis of 5,690 adults, with visceral fat measured by MRI and liver fat by spectroscopy in a subset, found visceral and liver fat mediate a large part of the SHBG to diabetes link. Strong evidence, still moving.

None of which requires anything dramatic. A low SHBG is a prompt to look at the markers sitting beside it on the same draw: fasting insulin, HbA1c, triglycerides. That is a question you can go and answer.

What High SHBG Means

A high SHBG binds up more of your testosterone, which can leave free testosterone low even when the total number looks perfectly respectable.

The Endocrine Society's list of what raises it is short and specific:

  • Aging

  • Hyperthyroidism

  • Cirrhosis and hepatitis

  • Estrogen use

  • Some anticonvulsants

  • HIV

  • Variants in the SHBG gene itself

High SHBG causes therefore split into things you cannot change, things a clinician should know about, and things in your medicine cabinet. High SHBG on its own is not a problem to be fixed. It becomes interesting when free testosterone is low and you have symptoms to match, which is what the next section is about.

What Moves SHBG: Insulin, Body Fat, Thyroid, Age, and More

SHBG goes down with obesity, diabetes, hypothyroidism, glucocorticoids, some progestins and androgenic steroids, and up with age, hyperthyroidism, liver disease, estrogens and some anticonvulsants. Thyroid appears on both lists because the direction matters.

The mechanism is the part most explanations skip, and it is why low SHBG carries metabolic meaning at all. Hepatic SHBG production responds to liver fat. In transgenic mice and in human liver cells, glucose and fructose cut SHBG output by suppressing a transcription factor called HNF-4α, and blocking fat synthesis in those cells prevented the whole sequence.

So what causes low SHBG is better answered with the liver than the pancreas, and the low SHBG causes on that list mostly run through it. Insulin's direct role is unsettled, and human data point more at fasting glucose and liver fat than at insulin itself. Both findings above come from animal and cell-line work, with no human study attached.

Which makes how to lower SHBG an uncomfortable question, since the reliable ways to do it are the ways nobody wants. How to raise SHBG has the better evidence anyway: in an umbrella review of meta-analyses, weight-loss interventions in men raised SHBG, with bariatric surgery producing the largest and most consistent effect on moderate to high quality evidence.

Why 'Normal Testosterone' Can Still Feel Like Low T

A high SHBG can leave you symptomatic with a normal-looking total testosterone, and a low SHBG can hand you a low-looking total testosterone while your free testosterone is fine. Both happen, and the second one is far more common than the internet suggests.

Take the first. The European Male Ageing Study, a random sample of 3,369 men aged 40 to 79 across eight European centres, proposed that late-onset hypogonadism needs three sexual symptoms plus a total testosterone below 11 nmol/L and a free testosterone below 220 pmol/L. Two numbers, because one could not separate the men with a problem from the men getting older.

Now the reassuring direction, which is where SHBG earns its place on the panel. In men carrying extra weight, the drop in total testosterone is driven largely by a drop in SHBG, itself downstream of hyperinsulinemia, high triglycerides and liver fat.

When testosterone and SHBG fall in proportion while LH and FSH stay normal, the axis itself is working. And in the EMAS follow-up, hypogonadal symptoms tracked the 27.3 percent of men whose total and free testosterone both declined; they were largely absent in the majority whose total fell while free stayed normal.

So before concluding anything from a single low number:

  • Draw it fasting and early in the morning

  • Draw it again, because up to 30 percent of low results normalize on a repeat

  • Ask for LH and FSH alongside total testosterone and SHBG

That combination is what tells you which story you are in.

How to Actually Read Your SHBG Result (Reference Ranges & Free Androgen Index)

Read your SHBG against the range printed on your own report, because published adult-male ranges genuinely disagree with each other. This is not a legal hedge. It is the finding.

US survey data produced an SHBG reference interval of 12.3 to 79.6 nmol/L for men aged 20 to 49, and 23.5 to 114.7 nmol/L for men aged 50 to 80, from a 1,477-person healthy reference cohort. The lower bound nearly doubles between those age bands, so any single figure offered as the SHBG normal range has already thrown away the age you are.

Assay variation compounds it, and the Endocrine Society puts a number on how badly. When 1,133 laboratories using 14 different assays measured total testosterone in the same sample, results came back anywhere from 45 to 365 ng/dL.

Free testosterone assays are worse, with no standardization at all, so the lower limit of "normal" moves between labs. Whatever your SHBG blood test says, the range beside it belongs to that lab and that assay.

Which brings us to the free androgen index. FAI is not a valid measure in adult men, and the reason is arithmetic. The index was built for female hirsutism, and deriving it assumes SHBG binding capacity vastly exceeds testosterone concentration, which holds in women and fails in men.

Measured against directly determined free testosterone, FAI correlated at 0.858 in 20 adult women and 0.435 in 19 adult men. The Endocrine Society's position is that extending it to males was never formally justified.

Use what the guidelines specify instead: equilibrium dialysis with mass spectrometry is the reference method, and where that is unavailable, free testosterone should be calculated from total testosterone, SHBG and albumin using an accurate formula. A calculated free testosterone built on high-quality total testosterone and SHBG assays is described as the most useful clinical marker available. It needs both inputs, which is the point.

One piece of scope, stated once: not everyone agrees on how much free testosterone adds, and the older calculation formulas have documented accuracy problems against equilibrium dialysis. What nobody disputes is that SHBG changes what a total testosterone means.

When SHBG Should Prompt a Bigger Conversation

SHBG tells you where to look next; it does not diagnose anything, and no single value settles a question on its own.

Three situations genuinely warrant a conversation with a licensed provider:

  • Symptoms alongside a low testosterone that stays low on a repeat draw

  • A low SHBG next to a raised HbA1c or fasting insulin

  • A high SHBG with real symptoms and an unremarkable total testosterone, the classic case for looking at free testosterone properly

Erectile difficulty is one of the three sexual symptoms in those EMAS criteria, so the hormonal causes of ED belong in this conversation too.

A provider can weigh all of it at once, including thyroid function, liver health and whatever else you are taking, since several common medications move SHBG both ways.

If a workup does establish low testosterone with symptoms to match, treatment is a decision for the provider who evaluates you, and the evidence behind axis-stimulating options comes from trials that enrolled men with a diagnosis rather than men chasing a number. Enclomiphene, for instance, is a compounded medication and is not FDA-approved.

What comes first is duller and more useful: a complete draw, repeated, putting SHBG next to total testosterone, LH, FSH and your metabolic markers.
